Remote Physiologic Monitoring (RPM) Expansion

The AMA introduced new RPM codes (such as 99445 and 99470) to accommodate clinical scenarios that do not fit traditional 30-day monitoring rules:

Short-Term Data Transmission (99445): Covers device supply and data transmission for 2–15 days of monitoring within a month, offering an alternative to 99454 (which requires 16+ days).

Low-Duration Management (99470): Covers 10–19 minutes of RPM clinical time per month, filling the gap before reaching the 20-minute threshold required for 99457.

Refined Criteria for Medical Decision Making (MDM)

While the fundamental E/M structure remains based on either MDM or total time, payers and auditors have tightened documentation enforcement:

Stricter "Problems Addressed" Verification: Simply listing a chronic diagnosis in the medical record or history no longer counts toward MDM complexity. The clinical documentation must explicitly show that the provider evaluated, treated, or managed the condition during that specific encounter.

Clarified Stability Rules: A condition is considered "stable" only if the patient is meeting clinical treatment goals. If treatment goals are unmet, auditors permit coding as an unstable or progressing chronic condition, justifying a higher MDM level.
